Please use this form to complete registration for the Level 2 BASIS-A Training. You may choose a single payment option or you can select to pay 3 separate installments. If paying installments, at least 50% of the total fee must be paid before the 2nd individual supervision session, with the final payment by the last group meeting.  Once you submit the form, Susan will contact you to schedule sessions.
